Beethoven is writing to his brother apparently referring to an enclosed letter (no longer surviving), which he had written long before. He hopes that the other brother (Kaspar Karl van Beethoven) can summon up more understanding as he would otherwise really suffer, particularly because he is dependent on others due to his bad hearing.
